Rural Maternity and Obstetrics Management Strategies Program

This notice announces the opportunity to apply for funding under the Health Resources and Services Administration’s (HRSA) Rural Maternity and Obstetrics Management Strategies (RMOMS) program. The purpose of the RMOMS program is to establish or continue collaborative improvement and innovation networks to improve access to and delivery of maternity and obstetrics care in rural areas. The goals of the RMOMS program are to: (i) Identify and implement evidence-based and sustainable delivery models for the provision of maternal and obstetrics care in rural hospitals and communities; (ii) Enhance and preserve access to maternal and obstetric services in rural hospitals that includes developing an approach to aggregate, coordinate, and sustain the delivery and access of preconception, prenatal, pregnancy, labor and delivery, and postpartum services; (iii) Provide training for professionals in health care settings that do not have specialty maternity care; (iv) Collaborate with academic institutions that can provide regional clinical expertise (such as specialty expertise and provider support using a variety of modalities including telehealth services) and help identify barriers to providing maternal health care, including strategies for addressing such barriers; and (v) Assess and address disparities in infant and maternal health outcomes, including among rural racial and ethnic minority populations and underserved populations. Applicants are encouraged to propose novel ways to achieve these goals through the establishment or continuation of collaborative improvement and innovation networks. The RMOMS program seeks to document and monitor progress on these goals through the collection of aggregate data from each RMOMS award recipient and their network members. All network members will be REQUIRED to collect and share aggregate data. This program intends to preserve access to and continuity of maternal and obstetrics care in rural communities that address the following RMOMS Focus Areas: 1) Rural Hospital Obstetric Service Aggregation A regional network with several rural hospitals that are facing challenges in providing obstetric services could aggregate obstetric services to a targeted rural hospital or Critical Access Hospital (CAH) within the rural region to revive or sustain rural obstetric and maternal services. 2) Approaches to Risk-Appropriate Care Pregnant patients in a rural region should receive care in a facility that best meets their needs and those of their neonates through appropriate risk stratification. 3) Financial Sustainability Rural hospitals who have coordinated and aggregated their obstetrics services, in partnership with Medicaid and other payers, should aim to demonstrate improved outcomes and potential savings with the goal of ensuring ongoing support of the network once federal funding ceases. Fed Dev Ontario supported WOCFDCA with capital investments in SOFII, as part of the agency’s Prosperity Initiative, originally in 2012 and subsequently in 2014 and 2019. The Rural Business-Cooperative Service (the Agency) is issuing a second Notice of Solicitation of Applications (Notice) under the Rural Energy for America Program (REAP) as referenced in the notice that was published in the . . This second notice announces the availability of $1.055 billion in Inflation Reduction Act funds across six quarterly cycles to be obligated by September 30, 2024. This Notice also announces the types of projects that qualify for a federal grant share not to exceed 50 percent of the project cost, a set-aside for underutilized renewable energy technologies (underutilized technologies), as well as scoring revisions to support Administration priorities. Applications received on or after April 1, 2023, will be evaluated and scored according to the provisions listed in this Notice, unless otherwise amended via a subsequent notice. The Notice will not be applied retroactively to any applications previously filed. Team Grant: Improving Diagnosis for Rare Disease Patients

In Canada, using current clinical diagnostic pathways, rare diseases (RDs) take an average of seven years to diagnose. This includes 2-3 misdiagnoses prior to the final correct diagnosis, with many specialist and subspecialist visits and much unnecessary testing. This diagnostic odyssey, along with social isolation, an absence of care coordination, and often high socioeconomic and mental health burdens, are frequently the norm for those living with RDs Compounding the situation is the fact that more than two thirds of RDs have a pediatric onset.. . The majority (>80%) of RDs are disorders caused by single-gene mutations. Advances in genomics have increased the number of RDs that can be identified, from 500 five years ago1 to over 5,500 of the 7,000 known genetic RDs at present2 with this number rapidly increasing. Although genomics as a diagnostic tool for RDs has emerged in the past few years as an option, it is often the last diagnostic test ordered once all other traditional possibilities have been exhausted. Nonetheless, there are inherent expected advantages to using genomics to diagnose RDs earlier in the care pathway. Using genomics early on is expected to reduce the time to diagnosis to 6 months, and consequently, significantly reduce the existing cost associated with the current “diagnosis odyssey”. In addition, genomics would not only facilitate increased precision of the diagnosis but could also potentially decrease the indirect socioeconomic and psychological impact on patients with RDs and their families. For instance, early diagnosis would enable access to therapies earlier in life, many of which could prevent lifelong disability (e.g., mental impairment). Importantly, not all RD therapies are expensive, and many can have life-changing consequences – if diagnosed in time.

Child and family well-being for all families, including families most impacted by racial disparities, is the responsibility of equitable, diverse, and culturally competent systems and the community.  The purpose of this NOFO is to fund grants that support the development, implementation, and evaluation of field-initiated approaches to addressing racial bias and inequity in child welfare; and improving the safety, stability, and well-being of families in traditionally underserved communities.As a field-based initiative, this funding opportunity will enable communities to design and lead targeted solutions to local issues of systemic disproportionality and inequitable access to services experienced by traditionally underserved communities at varying points across the child welfare continuum.Grant recipients will utilize supporting data, and any pre-analysis if available, from their respective states or communities that describes the problem of inequity and racial bias as it exists in their community. All Canadians, across the lifecycle, are affected by the quality of the air we breathe, both indoors and outdoors. Inhalation of gases and toxins resulting from, but not limited to, smoking, vaping, radon and air pollution can significantly impact the health of the lungs and other organ systems, such as the heart and brain. Resultant absorption of particulate matter and aerosol pollutants can damage lung tissue directly and cause local and systemic inflammation throughout the body. Considering the ongoing, new and emerging threats to lung health, such as lung cancers among people who have never smoked, the effects of vaping on health, viral infections (e.g., SARS-CoV-2), and the impacts of climate change, there is an urgent need for strategic investment in lung health.. . Across the lifecycle, particularly at the extremes of age (the very young and very old), individuals are vulnerable to the impact of poor air quality. Poor air quality (through air pollution, climate emergencies, and atmospheric inversions) has been linked to preventable emergency department (ED) visits, hospitalizations and even deaths. A diverse range of serious cardiovascular and respiratory conditions have been shown to increase within three days of increases in ozone, sulfur dioxide, nitrous oxide and carbon monoxide as measured through the Air Quality Health Index (AQHI). The pathophysiology of these resultant conditions is thought to be related to systemic inflammation, so it is perhaps not surprising that other inflammatory conditions such as stroke, heart attacks, infections, and headache can increase with poor air quality measures.. . Moreover, cigarette smoking and vaping are estimated to contribute significantly to preventable deaths, hospitalizations and ED visits. The recent dramatic rise in vaping among youth who have never smoked, along with the E-cigarette and Vaping Products Acute Lung Injury (EVALI) crisis in the US and Canada prior to the pandemic, reflect the need to continue examining the health effects of vaping. While cigarette smoking has been linked to lung and multiple other cancers, lung cancers in people who haven't smoked are increasing, and the role of lung toxins in the etiology of these cancers requires additional investigation. Over time, the accumulated toxic load on the lung from ongoing exposure to environmental pollutants, such as smoking, vaping and poor air quality, becomes unsustainable and result in detrimental health effects on the lung. For individuals with underlying and/or active lung disease, these exposures result in increased symptom burden and morbidity.. . Finally, there are inequitable impacts on lung health connected from geographical location, social status, ancestral heritage, sex, gender, sexual orientation and age. For example, it is becoming more common for people residing in remote, rural and northern communities to be adversely affected by poor air quality caused by wildfires. Women generally are both under-represented in research and mis-diagnosed for cardio-respiratory conditions, suggesting that sex and gender considerations are in urgent need of attention. Moreover, Indigenous Peoples (First Nations, Inuit and Métis) in Canada are at risk of greater exposure to toxins in their lungs due to higher smoking rates, crowded/poor housing and environmental factors that affect air and water quality. Interestingly, current evidence suggests there are wide differences between First Nations, Inuit and Métis populations related to rates of detection for various lung health conditions.. .
